First, have you heard the good news about Jesus? If you have, do you believe in Him? Do you believe He is the Son of God and He died, was buried, and rose from the dead? Have you repented of your sins, meaning have you taken a quality turn in the opposite direction of your sins?

If you have answered yes to all these, then you are ready to be baptized. A Christian man who has already been baptized can take you to any body or tub of water big enough to dip yourself into. He will ask you "Do you believe that Jesus Christ is the Son of God and are you ready to make Him Lord of your life?" If you respond yes, he will say "I baptize you in the name of the Father, the Son, and the Holy Spirit for the remission of your sins." He will then dip you completely into the water and raise you back out.

Note that Scripture does not have any exact words or posture of how to baptize someone, but this is what is typically done, and the words are based on Scriptures.
